Post-assessment feedback survey

The DAP encourages feedback about the accreditation services it provides to diagnostic facilities.

Following each assessment, an online survey is provided to the facility with questions spanning the entire accreditation process. The feedback provided is used to continually improve the assessment experience.

Filing a complaint regarding DAP policies or practices

Beyond the post-assessment feedback, diagnostic facility personnel may file a complaint with the DAP regarding policies or practices.

The DAP will respond to all complaints within 30 days. All complaints are handled as swiftly as is reasonably practical depending on the nature and complexity of the investigation.
