A facility must read and/or complete the following forms prior to an initial assessment.
Scheduling the on-site assessment
An accreditation specialist will contact the facility to schedule an on-site assessment after the DAP has received and reviewed the documents.
Sample collection and POC testing facilities should complete this form for review by the DAP
