Scheduling the on-site assessment
An accreditation specialist will contact the facility to schedule an on-site assessment after the DAP has received and reviewed the documents.
DAP accreditation is required for COVID-19 rapid antigen, PCR or serology testing
Sample collection and POC testing facilities should complete this form for review by the DAP
