Our forms are moving!
If you clicked a form link and arrived at this page, or are looking for a form that is no longer publicly available, log in to the new accreditation software (Fabric) to access the new processes and online forms.
Forms are transitioning from the website to Fabric between now and mid-October
We appreciate your patience as we complete this transition. Please note that after October 1, the accreditation programs will no longer be accepting forms or updates by email.
Contact your facility medical directors and assigned delegates if you require access to the new software. Your facility medical director or assigned delegate can reach out to accreditation.project@cpsbc.ca
What is the new software?
ARMATURE Fabric is a cloud-based accreditation software.
CPSBC’s version of Fabric is tailored to the unique activities required for DAP and NHMSFAP accreditation and monitoring. It is designed to capture records, usernames, and dates for accurate accreditation documentation.
Fabric will be used for:
- collecting pre-assessment information and evidence
- performing the assessment
- receiving assessment reports
- responding to nonconformances
- facility reporting of significant changes in scope, personnel or activity
- facility reporting of patient safety incidents, changes in privileged physicians, and program-specific quality control and quality assurance reporting
- maintaining current contact details
CPSBC’s information technology, legal, and privacy teams have reviewed Fabric to confirm that it is secure and compliant with provincial and national legislation and regulations, and CPSBC’s record management and retention requirements.
Fabric does not retain records in the system. Instead, records are transferred to a secure repository for appropriate storage and retention.
If you have any questions about privacy and/or how CPSBC manages records, contact privacy@cpsbc.ca.
The current combination of software and systems no longer meets the needs of the accreditation programs.
Switching to Fabric will help centralize activities into one accessible platform for facility medical directors and staff. It also enhances transparency during pre- and post-assessment activities.
Fabric will become the primary communication tool between facilities and the accreditation programs. It will replace most email exchanges and allow all assessment activities and updates to be tracked in one single place.
