Medical Identifier Number for Canada (MINC) Consent Form
A MINC number is a unique lifetime identifier assigned to each individual across Canada who enters postgraduate training or practice in this country.
 
Temporarily Inactive – Parental Leave
For use by physicians to inform the College that they will be taking a parental leave from practice.

Request (by an individual) for Certificate of Professional Conduct

Health Professions Review Board

The Health Professions Review Board (HPRB) is an independent tribunal tasked with reviewing registration decisions. If an applicant is dissatisfied with the Registration Committee's decision, s/he can submit the matter to the HPRB for further evaluation and review of the College’s action and conclusions within 30 days of receipt of the decision letter from the Registration Committee.
